To complete your request for a personal consultation, please complete your payment information below. ADD Consults accepts payment by MasterCard, Visa, Discover, or American Express. If you wish to make arrangements to pay by check, please contact Terry.

Your card will be billed for an initial consultation fee in the amount of $100.00 US. Once your credit card has been approved, you can expect a personal response from Terry within 24-48 hours.

How did you hear about ADD Consults?

The fee for an initial e-mail consulation is $100.00 US, which includes up to two e-mail responses. Phone or chatroom consultation fees are $100.00 US for 45 minutes. Payments may be made online by MasterCard, Visa, Discover, or American Express.
